Summer Information
- OHSAA Physical Form
- Must be filled out by a doctor and uploaded to FinalForms.
- Valid for 1 year from the exam date
- CANNOT ATTEND TRYOUTS WITHOUT A VALID PHYSICAL

- Parents and players must create an account.
- The first scrimmage must fill out all athletic department forms.
- Success Beyond the Classroom Scholarship
- Scholarship to cover the Pay to Participate fee for those unable to cover it.
- Due to the athletic office by 3:00 pm on July 25, 2025.
- Summer Dates
- Open fields and conditioning: Monday, Wednesday, and Friday from 7 am – 9 am.
- Big Walnut Showcase: Friday, July 25 and Saturday, July 26. INVITE ONLY.
- Mandatory Tryouts
- August 1st and 2nd from 7 am – 9 am.
- Team selection meetings: August 2nd from 5 pm – 6:30 pm.
- Beginning August 1st, ALL practices, games, and team events are MANDATORY.

- All team communications come from TeamSnap. Please ensure that you and your athlete have connected to the team, downloaded the app, and enabled notifications.

FinalForms is an online system used by our athletic department to manage student-athlete registration, medical forms, and parent/guardian permissions. All athletes must complete FinalForms before participating in tryouts, practices, or games. Please use the links to access the Parent Playbook in your preferred language for step-by-step instructions.

The OHSAA (Ohio High School Athletic Association) Physical Form is required for all student-athletes to participate in school sports. It must be completed by a licensed medical professional and submitted before tryouts. Download the form below in English or Spanish.
Success Beyond the Classroom Scholarship

SBC’s goal is to help SWCSD students in need of assistance with their participation fees by awarding as many scholarships as possible each season of each school year. SBC scholarships are awarded in varying amounts, covering up to 75% (maximum) of the corresponding participation fee. Students can apply for a SBC scholarship by completing an application and returning it to their school’s athletic director or mailing it directly to SBC before the established due dates indicated on the application.
